Here's the form to use 

Your email is not added to any sort of list. When the contact window pops up just click "Contact" if an email field isn't showing.

Note: The links pop up a small window where you can fill you the form. If you don't see it appear then you may be running an ad or script blocker, such as the one used by the Firefox browser. You'll have to enable scripts for this site to see the contact form.
